HOW TO REGISTER WITH THE PRACTICE

REGISTER WITH PRINGLE STREET SURGERY

This will automatically be sent to our reception team for processing. If we require further information, our reception team will be in contact with you. 

You do not need photo ID or proof of address, anyone is welcome to register at our practice but it would be helpful to our registration process if you could present the reception staff with proof of ID (Passport, Driving license or some kind of photo ID).

A medication review will be carried out for those taking drugs of dependence (e.g. sleeping tablets, morphine derivatives, gabapentinoids) and steps will be taken to help you wean off these medications.

If you are a carer, veteran, or require reasonable adjustments, please let us know when registering.

CHANGE OF NAME/ADDRESS/TELEPHONE

We would be grateful if you could inform the surgery of any change of your details as soon as possible in order to keep our records up to date. This helps prevent NHS letters being lost and problems in contacting you, especially in emergencies.

PATIENTS UNDER 16

It is requested that patients under 16 are accompanied by an adult. 

New babies should be registered with the Practice as soon as possible.
